Providers: real Claude Haiku 5.5 and Codex gpt-6.1-sol via VM local proxy.
No mocked provider or replay/demo data.

Observed in UI and MCP server log:
main empty form: immediate decline without approval card.
PR empty form: card waits for click; Approve -> accept with content {}.
PR second call: fresh card; Decline -> decline.
PR third call: Cancel menu -> cancel.
PR email-required form: decline without card.
PR enum with scope default always and persistence toggle default true:
  Approve -> scope allow_once, remember false.
PR pending request + Stop generation -> cancel; card removed; reload stays stopped.
Codex full-access mode: automatic decline.
Codex supervised: tool permission card then elicitation card;
  Approve -> scope allow_once, remember false.

Regression found in original PR head:
required enum [disallow, allow_this_time] declined without card.
Existing test table extended with allow_this_time failed before fix.
Fixed exact matcher preserves this_time spelling while still excluding disallow.
After dependency install and server restart:
  real Claude Approve -> accept {scope:allow_this_time}.
  real Codex supervised Approve -> accept {scope:allow_this_time}.

  259 passed before additional regression case.

  format, lint, knip (both), graphql, affected typechecks (2 packages), 259 tests passed.
Explicit base used because the remote snapshot lacks origin/main ancestry by default.

Follow-up review regression: allow_once_only declined without elicitation card on both real Claude and Codex paths at fc4f4b7572. Existing test table row failed before fix. Optional only suffix after once added; no new test files. After install and server restart, both providers showed the card and returned allow_once_only only after Approve. Original empty form also retested successfully on cef662fe0e.
Final Blacksmith check:changed passed all scoped checks and 259 tests (log /tmp/pr21903-check-final.log).
