Project and all threads created through web UI; real git repository; no mocks or seeded conversations.

Project PermissionRequest hook returns behavior=allow; full-access thread.
Main observation before stopping server: request= pending, resolvedAt= null; UI still Waiting for approval after successful provider result. Main server shutdown subsequently cancelled it at 03:51:56.197Z; the current rows below include that shutdown cleanup.

Web observations: main still showed Waiting for approval after successful tool output. PR had no pending card and returned to send control. Approve executed accepted-a / accepted-b. Decline prevented the command and did not retry. Codex gpt-6.1-sol completed terminal printf with codex-neighbor-ok.
Blacksmith: ClaudeAdapterV2.test.ts: 256 passed. CI: no failing or pending jobs at inspected head; GitHub reports 8 skipped jobs.
Not exercised: native desktop/mobile, remote/relay/tunnel, live AskUserQuestion withdrawal, approval-answer/withdrawal race.
